This is an important reminder from your child’s school nurse. Make sure to complete the required 7th grade physical and immunizations prior to the start of the school year. Due to construction at Horizon Middle School do not bring them to the school over the summer. You may turn them in at Horizon Middle School registration on August 4th or 5th. If you previously handed in the completed physical and immunization paperwork to your child’s school, please disregard this message. State law requires proof of a 7th grade physical and 7th grade immunizations (Tdap), prior to a child starting 7th grade. We need this paperwork as the child cannot enter the classroom without it being reviewed by the school nurse ahead of time. We don’t want incomplete paperwork to delay your child’s first day of 7th grade!
If you have not made an appointment with your child’s medical provider, please do so today as appointments fill up quickly.
If you do not have health insurance or Medicaid for your child, contact the newly opened HelpCare Clinic, the first week of June, to see if your child qualifies for a free physical and immunizations (phone 308-224-2392). School physicals will be conducted on June 24, July 8 and July 10 from 9am to noon. Immunizations will provided on these dates through Mid NE Community Action Partnership, suggested price for immunizations is $18. Students needing immunizations need to bring a record of prior immunizations with them.
Thanks and have a great summer!
Your Child’s School Nurse

Renovation to Change Student Drop-off and Pick up Procedures
Beginning March 9, Horizon EAST parking lot will be closed.
Construction fences will be in place to keep traffic out.
PICK UP AND DROP OFF CHANGE
The Horizon parking lot to the EAST will become the staging area for construction equipment. Some students were picked up or dropped off in this location. This will no longer be an option. People are encouraged to drop-off students in front of Horizon Middle School along the north side of 35th Street or along 11th Avenue on the west side of Horizon.
BUS HUB LOCATION CHANGE
The bus hub will move to the east into the high school parking lot. This change will take place on March 9th and will continue until all of the construction at Horizon is complete.
Thank you for your patience during all of the construction.
